In order to ensure the timely processing of your application, please submit your application form with all the supporting documents required and a signed copy of Subscription Terms and Conditions to any of our local offices . NOTE: The submission of all or any of these documents does not guarantee that your application will be successful. We reserve the right to seek additional information regarding your application from third parties. Assessment of applications are based on several factors including your ability to pay the monthly subscription fees on time, access to continuous source of electricity, age and accessibility of SKY technicians or our agents to the installation location, among others. You are also NOT required to make any payments at this stage. Our customer service representatives will contact you with the outcome of your application shortly after it has been submitted to us with payment options and other information relating to your application.
Other Application Forms
SKY Account Transfer Form is used to transfer account ownership of existing SKY Pacific account holder to a third party.
SKY Account Type Changeover Form is used to request for your existing SKY Pacific account to be upgraded or downgraded from the current package or subscription plan.
SKY Change in Account Details Form is used to amend your existing SKY Pacific account details with us excluding the change of "Name" of the account. Details that can be amended via this form includes billing and installation address and contact details of the subscriber. To change name of the Account, please see " SKY Account Transfer Form".
SKY Disconnection Request Form is used to make request to temporarily or permanently disconnect your SKY Pacific account.
SKY Refund Form is used to make request for refund of payments made to SKY Pacific.
SKY Credit Card Authority Form is used to authorise SKY Pacific to make a payment via your credit card. Payment authority is accepted for Master and Visa credit cards through this form.
